• desired service/treatment direction;
• other information voluntarily provided by the User in the request form (e.g., preferred appointment time).

List of actions performed with personal data for Purpose No. 1: collection, recording, systematization, accumulation, storage, clarification (updating, modification), extraction, use, transfer (provision, access), blocking, deletion, destruction — carried out as mixed processing, without transfer over an internal network, with transfer over the "Internet" network (including through the Site's form and the Company's email/CRM services).
